Position Summary:

 

IPH Medical Respite is a 17 bed emergency shelter for homeless adults discharged from hospitals or other acute care settings, with services focused on improved health outcomes, health education, and connections with health care services.  Each guest receives strengths-based and comprehensive services and support addressing a unique set of needs.  Medical Respite provides a supportive setting where guests can focus on overall health while addressing other factors that have led to their homelessness.

 Guest Support Specialist 

Summary

IPH’s Community Connections in Albany serve individuals experiencing a broad range of challenges, including poverty, mental health and substance use disorders, medical challenges, and legal issues.

IPH is currently hiring a per diem Guest Support Specialist for Community Connections. IPH’s Guest Support Specialist Supports the operation of Community Connections and provide access to services. This position reports to the Community Connections Supervisor.

Case Manager

Position Summary:

IPH is currently hiring a full-time Case Manager for all Community Connections. The Case Manager is responsible for providing efficient and effective service delivery. Community Connections serves individuals in the community experiencing current homelessness, at risk of facing homelessness, and community members who are housed with limited resources.

Summary

IPH’s Emergency Shelter and Medical Respite Shelter in Albany serve individuals experiencing a broad range of challenges, including poverty, mental health, and substance use disorders, medical challenges, and legal issues. IPH’s Direct Support Staff oversees the operation of the shelters and provide supportive services to shelter guests on the weekends and outside of regular business hours.

Direct Support Staff are responsible for maintaining an emotionally and physically safe and supportive environment for shelter guests
